Training Module Description: 

Recognizing that the majority of sepsis cases begin outside of the hospital, it’s important for home healthcare providers to be well equipped to recognize sepsis. This training module reviews sepsis identification and patient care for home healthcare providers. Patients are cared for in the home following various health conditions, illnesses, and surgery. Many of these patients have chronic conditions putting them at risk for infection and sepsis. Home health providers have a unique opportunity to identify sepsis in the field. The sepsis screening process is discussed and the Sepsis Home Healthcare Detection Tool is presented. A sepsis survivor shares their experience with home care and hospitalization. This activity will help the home care team learn to screen for and detect possible sepsis.

Target Audience

Home care nurses, home health aides, physical therapists, occupational therapists, speech language pathologists, medical social workers, and mental health staff.

Learning Objectives

At the end of the course, the learner should be able to:

  • Define sepsis
  • Discuss risk factors for sepsis
  • Identify individuals who meet criteria for sepsis using a sepsis screening tool
  • Describe key elements of timely interventions during transitions of care

Al Cardillo, MSW

President and CEO, Home Care Association of New York State

Al Cardillo is the President and CEO of the Home Care Association of New York State (HCA), a statewide organization of over 300 home care agencies, hospices, long-term care plans, waiver programs, allied organizations, and individuals providing home care to several hundred thousand New Yorkers annually. He has served in multiple roles: Executive Director for Health in the NYS Senate, Director of the Senate Health Committee and the Council on Health Care Financing; Coordinator for Long Term Care in the Senate; Coordinator for health legislative task forces; and program development staff for the NYS Division of Medical Assistance implementing home health care programs in NYS.

Amy Bowerman, RN, BSN

Executive Director of Senior Network Health, Mohawk Valley Health System

Amy Bowerman is the Executive Director of Senior Network Health MLTC and the Director of Quality Improvement and Privacy Officer for Visiting Nurse Association of Utica and Oneida County.  Both organizations are part of the Mohawk Valley Health System in Utica, NY.   Amy serves as a Board Member for the NYS Home Care Association, as well as sitting on the HCA Quality Committee, MLTC Forum, and is the SEPSIS workgroup clinical leader.  Amy brings over twenty years of nursing experience in Emergency Nursing, Home Care, Pediatrics, Quality, Nursing Administration and Managed Care.
